If you plan to dive at Ginnie Springs, or take part in one of our diver training programs, you can save time by downloading, completing and signing the necessary forms ahead of time.

Scuba Diving Waiver release (Adult)

The is the two-page form that all adult scuba divers must sign and have witnessed prior to diving at Ginnie Springs (students must have their instructor complete the "Witness" portion). Please note that this is a two-page form; you must complete and sign both pages.

Scuba Diving Waiver release (Minor)

The is the two-page form that all scuba divers under the age of 18 must have a parent or legal guardian sign prior to diving at Ginnie Springs (students must have their instructor complete the "Witness" portion). If the parent or legal guardian will not be accompanying the minor during training, the parent or guardian must have his or her signature on the Ginnie Springs Waiver notarized. Please note that this is a two-page form; you must complete and sign both pages.

RSTC Medical Statement

This is the form that participants in Ginnie Springs' Cavern and Intro to Cave Diver courses must complete and sign prior to the beginning of training. You must answer the questions appearing on the Medical Statement by writing the words Yes or No (no check marks or X's). If you answer Yes to any question, you must also obtain the five-page Medical Exam form and Guidelines for Recreational Scuba Diver's Physical Exam (see below).

If you answer Yes to any question on the RSTC Medical Statement, you must also download this five-page Medical Exam form and Guidelines for Recreational Scuba Diver's Physical Exam and take them to your doctor. Your doctor must then read the guidelines and decide whether or not he or she can give approval for you to participate in diver training. If your doctor approves, he or she is to complete and sign the PADI Medical Exam form, which you will submit to us, along with your Liability Release and Medical Statement. (Note: If you have already had a medical exam for diving within one year of the start of training, and had a physician give his or her approval at that time, you may bring a photocopy of the RSTC Medical Exam form your physician signed at that time, in lieu of obtaining a new form.)
